Comprehensive Wellness Assessment
Comprehensive assessment + 3 follow-up sessions
Three months of personalized support to help you understand your health, focus on the priorities that matter most, and turn your plan into changes you can actually sustain.
We begin with a Comprehensive Wellness Assessment, looking at your health history, current concerns, symptoms, lab results, nutrition, sleep, movement, stress, medications, supplements, and goals.
From there, we identify your priorities and build a practical plan. Over the next three months, follow-up sessions give us time to see how your body responds, work through challenges, refine your approach, and keep you moving forward.
Who This Is For
The 3-Month Wellness Program may be a good fit if you:
-
Want more than a one-time assessment and written plan
-
Have health markers, symptoms, or changing health needs you want to work on
-
Are navigating metabolic or cardiovascular concerns, digestive issues, midlife changes, cancer treatment or survivorship
-
Want to take a more proactive approach to healthy aging
-
Know what you should be doing but need help deciding what matters most and putting it into practice
-
Would benefit from ongoing guidance, focus, and accountability
What's Included
Comprehensive Wellness Assessment
A 75–90 minute initial session covering your health history, concerns, available labs, nutrition, lifestyle, medications, supplements, and goals.
Personalized Written Plan
Clear priorities and recommendations based on what we uncover together.
Three 45-Minute Follow-Up Sessions
We'll review progress, discuss what you're noticing, troubleshoot challenges, and adjust your plan as we learn more.
Ongoing Support
Reasonable email support between sessions for questions related to your plan.
How the Three Months Work
We typically reconnect about two weeks after your initial assessment, then space subsequent sessions farther apart as your plan develops.
This gives you enough time to put recommendations into practice and see how your body responds, while still providing support early enough to make adjustments when needed.
The process is personalized. We may work on nutrition, metabolic health, digestion, movement, sleep, recovery, herbs, supplements, or other supportive strategies depending on your priorities.
The Space Between Knowing and Doing
Health information is everywhere. The challenge is figuring out what applies to you, what deserves your attention now, and how to turn that information into something useful in daily life.
Throughout the program, I help you stay focused on your priorities, make sense of what you're learning, and adjust when something isn't working. You have someone alongside you to provide perspective, guidance, and accountability as your plan evolves.
Scope of Practice
Carrie Amy Wellness provides education and wellness support and does not diagnose medical conditions, prescribe medications, or replace care from your physician or other licensed healthcare providers.
When appropriate, I can help you organize questions, observations, and relevant information to discuss with your medical team.
Investment
$575
The Comprehensive Wellness Assessment and all three follow-up sessions are included.
If you're not sure whether you need ongoing support, you can begin with the Comprehensive Wellness Assessment on its own and decide afterward whether additional sessions would be useful.
What happens after three months?
You can continue working with me based on the level of support that makes sense for you. Some clients continue with regular sessions, while others check in periodically as new goals, questions, or health changes arise.
